– Date of birth

– Voter registration and political party affiliation is optional.

Provide your current home address instead of P.O. box because it defines your voting districts like the polling station. Lastly, ensure that you have signed and dated the form when you are through with the completion of the form. This data will be applied to verify your qualification for the program or service.

If through mail, then the application is often required to be postmarked or physically submitted at least one month before the election. Read the form instructions concerning the proof of ID and other verification procedures, if any.
